Three forms may be downloaded at this site:

• The Letter of Recommendation Request Form contains information about obtaining letters of recommendation in general and specifics about requesting letters from a particular teacher.
• The Parent Questionnaire is an opportunity for parents to provide additional information to counselors for possible inclusion in the counselor letter to colleges. Appropriate information might include special talents or abilities that counselors might not know of or any extenuating circumstances that may have affected academics.
• A Transcript Request Form must be completed for each college and scholarship application. Students may either type their information into the fillable Transcript Request Form and print or the Transcript Request Form may be downloaded and completed neatly by hand. All forms must be brought to Mrs. Schrader in the Guidance Office. Please make sure that all blanks on the form are filled in and that the college/scholarship listed at the top is the same one listed at the bottom of the form.
